Agency Programs and Services

The People Concern is the largest provider of housing and social services on the Westside of Los Angeles County. The People Concern provides comprehensive, integrated services to those most in need of help. The People Concern clients include people dealing with mental illness, physical illness or disabilities; women and children experiencing domestic violence; and Veterans. The People Concern tailors services to the needs of each person to help them rebuild their lives. Key services include housing, mental health care, medical care, domestic violence services, substance abuse treatment, income assistance and life skills programs --- all aimed at improving the self-sufficiency of the individuals and families served.

SUMMARY

The FSP Clinician is responsible for engaging members and coordinating mental and recovery services that will ensure housing stability and community integration. The FSP Clinician is responsible for maintaining the highest quality provision of direct individual and case management services.

ESSENTIAL DUTIES AND RESPONSIBILITIES



  1. Provide, coordinate, and implement mental health and recovery support services with individual members.
  2. Develop effective, trusting relationships with clients experiencing tri morbid disorders, as well as the forensic population, with a focus on facilitating independence and maintenance of improved physical and mental health
  3. Collaborate with each member to develop their individual service plan; review and update it upon completion of goals
  4. Provide linkage to stable housing, and other supportive services as needed to assist clients in obtaining housing stability, in addition to client advocacy, case management, and benefit establishment
  5. Conduct screening interviews, complete housing documentation, and coordinate clients' move-in with the housing authority, property managers
  6. Perform initial comprehensive case management assessment and develop a plan of intervention that addresses their barriers, increases their income, and assists them in maintaining permanent housing
  7. Encourage and promote an environment that is strength-based to assist clients in meeting their individual goals
  8. Identify appropriate permanent housing options for clients, such as, subsidized housing Section 8, Shelter Plus Care, VASH, permanent supportive housing, affordable and market-rate housing, and other housing opportunities
  9. Work cooperatively and cohesively with other members of the FSP team, including participation in weekly staff meetings, group clinical supervision, and trainings
  10. Develop and maintain a complete, accurate, and current member file with all funder-required documents



QUALIFICATIONS



  1. Active BBS Licensure - LCSW, LMFT, LPCC, or PsyD.
  2. Must be registered with the Board of Behavioral Sciences
  3. LPS designated or will become LPS Designated in order to initiate 5150 involuntary psychiatric holds.
  4. Willingness to participate in the supervision of program interns
  5. At least two years experience working with clients with mental illness, chronic health issues, and substance use disorders.
  6. Demonstrated knowledge of clinical case management
  7. Knowledge of community resources
  8. Able to work in a high-tolerance, harm-reduction model with individuals who have multiple barriers
  9. Strong interpersonal and team-building skills
  10. Computer literate; able to effectively use computerized database for client file management, with basic skills in Microsoft Word, Outlook, and Excel programs.
  11. Current, valid CA Driver's license, reliable car, auto insurance, and an acceptable driving record.
  12. Able to obtain and maintain CPR/1st Aid certification
  13. Experience working with homeless individuals who are living with mental illness, substance addiction physical health issues, and forensic population preferred
  14. Minimum 30 WPM typing speed preferred
  15. Bilingual Spanish preferred



WORK ENVIRONMENT



  1. Combination of field and office environment
  2. Regularly required to sit, stand, bend and occasionally lift or carry up to 35 lbs.
  3. On occasion walk or drive to different local sites
  4. Will necessitate working in busy and loud environments
  5. Will be exposed to elements like cold, heat, dust, noise and odor
  6. May need to bend, stoop, twist , and sit throughout the day
