Essential Resident and Family Life Manager Duties/ Responsibilities:



  • Provides advocacy and assistance to all IL residents.
  • Will conduct psycho-social assessments and manage psycho-social concerns.
  • Will coordinate care and management of residents' overall well-being needs.
  • Will work closely with the clinical team and the Embrace Health Resident and Family Life Manager for transition in level of care cases.
  • Will work on developing, implementing and monitoring program goals and objectives needed for Southminster's senior adult community.
  • Acts as liaison to post hospital providers and insurance case managers. Coordinate proper placement post hospitalization and work closely with the Interdisciplinary Team.
  • Maintains contact with the resident and, as appropriate, his/her social support system to facilitate the members' participation in community life.



Qualifications

Education required:

Minimum bachelor's degree in social work and/or Gerontology.

Experience required:

Must have at least 3-5 years' experience in long term care.

Professional license or certification requirements:

Certified in Care Management preferred

Specific skills, knowledge and abilities required:

This person must have excellent communication skills, be energetic and self-directed.
