This Senior Patient Navigator will be a key position on our integrated behavioral health team in pediatric primary care, helping to support various innovative behavioral health initiatives. This Sr. Patient Navigator will be responsible for:
- Facilitating, tracking, and following up on community-based behavioral health referrals in close collaboration with primary care-based behavioral health clinicians
- Connecting with patients and families to provide support and guidance around the behavioral health referral process, as well as the implementation of ongoing care plans
- Collaborating with other behavioral health team members to track and monitor referrals in order to help ensure continuity of care and mitigate risk
- Coordinating logistics for plan adherence, such as appointment reminders and arranging transportation
- Explaining hospital/community-based resources to patients and families; providing documentation
- Acting as a liaison with internal and community resources. Contacting hospital and community resources to ensure access to services
- Initiating, developing, and maintaining relationships for accessing community-based resources
- Providing outreach and maintaining positive working relationships with providers/administrative staff (including physicians, office staff, nurses, and behavioral health staff)
- Reporting outcomes of referrals to the referral source
- Documenting patient care
- Collecting required data and developing and maintaining databases
- Tracking outcomes and supporting program development and strategic planning processes
- Analyzing data and summarizing findings
- Providing information to support community partnerships and grants
- May recommend program or procedural changes to improve outcomes or efficiency
- Attending and participating in case review and care planning meetings
- Disseminating training materials or ongoing education to more extensive program staff
- Serving as a subject matter expert to Patient Navigators, Interns, Social Workers, Psychologists, and others on clinic and specialized services offered in the clinic
In order to qualify, you must have:
- High School Diploma/GED required;BSW, undergraduate degree in psychology, or LMHC preferred
- A minimum of two years of experience, including direct experience with the patient population and community resources
- Strong interpersonal skills, including an ability to communicate effectively both orally and in writing and provide empathy in difficult interpersonal situations
- Fluency in Spanish may be preferred
- Analytical skills to resolve problems that require the use of basic scientific, mathematical, or technical principles
- Working knowledge of patient populations and the hospital/community resources that offer services and resources to meet patient, family, and/or clinic needs.
