EXPERIENCE AND EDUCATION

Required
  • Education

  • Bachelor's Degree from an accredited nursing program
  • Experience

  • 6 years of general nursing experience
  • Licenses and Certifications

  • (RN) REGISTERED NURSE - current licensure as Registered Nurse by Board of Nurse Examiners for State of Texas Upon Hire

    (BLS) BASIC LIFE SUPPORT course accredited by the American Heart Association (AHA) or American Red Cross (ARC) Upon Hire

JOB DUTIES
  • Supervises and supports patient navigation and patient centered quality improvement projects. Coordinates, performs and ensures activities are completed, develops strategies for continued growth and promotes efficiencies. Identifies resources, completes project documentation and ensures deliverables are met. Completes all required reporting accurately and in a timely manner.
  • Supervises and manages day to day activities for patient navigators, identifies best practices for patient navigation and staff challenges. Addresses challenges and promotes possible solutions.
  • Works in conjunction with individual departments and health system leadership to build awareness of patient navigator services, develops policies and procedures for the patient navigator team.
  • Oversees and evaluates the performance of patient navigators, supervises and facilitates development of the patient navigators. Responsible for hiring, performance evaluations, and disciplinary actions. Ensures the new employees are oriented and trained appropriately.
  • Designs and develops a formalized evidence based patient navigation program that includes but is not limited to addressing health disparities, psychosocial aspects of disease process and care coordination. Develops course materials, tutorials, and competency testing.
  • Works as a team member and promotes teamwork with the patient navigation stakeholders (care coordinators, social workers and faculty).
  • Duties performed may include one or more of the following core functions: (a) Directly interacting with or caring for patients; (b) Directly interacting with or caring for human-subjects research participants; (c) Regularly maintaining, modifying, releasing or similarly affecting patient records (including patient financial records); or (d) Regularly maintaining, modifying, releasing or similarly affecting human-subjects research records.
  • Performs other duties as assigned.
