Posting Summary
Rutgers, The State University of New Jersey, is seeking a Patient Navigator for the department of Neurosurgery at the Robert Wood Johnson Medical School.

The Patient Navigator serves as the primary point of contact to providers, patients and families for the navigation of neurosurgical patients according to specialty to meet the departmental program needs. The navigator will educate and guide patients through the health care system, ensuring appropriate appointments, for clinical visits and tests, are given in a timely manner through guidelines agreed upon with the department's management. The navigator will assist with access issues, develop relationships with referring health care providers and ensure the complete and efficient management of all referrals, including the acquisition of necessary patient-care materials and medical records. In addition, the navigator will track all new patient requests, provide monthly statistics and referring patterns.

Among the key duties of this position are the following:

  • Provides information and education to patients about services, expertise and care management.
  • Directs and implements care coordination plans in both inpatient and outpatient settings.
  • Obtains patient clinical information to allow for appropriate scheduling of patient appointments and referrals.
  • Communicates with physicians, patients, family members or patient representatives to establish patient's plan of care, goals and expected outcomes.
  • Assists physicians with all coordination aspects of patient care.
  • Verifies insurance eligibility and provides guidance around referral and managed care issues, and information about other prior authorizations that may be needed for testing and procedures.
  • Follows patients throughout care management process to verify that they have follow-up appointments with appropriate departments.

Minimum Education and Experience

  • Bachelor's Degree in a relevant field, plus two (2) years of relevant experience in a health care environment.
  • Equivalent experience, education and/or training may be substituted for the education requirements.

Physical Demands and Work Environment

  • Physical Demands: Computer literacy, typing, working at multiple workstations in a shared environment, ability to speak clearly, write proficiently, extensive reading, extended sitting, ability to move from unit to unit. Position requires standing and walking over 2/3 of the time. Talking, hearing & sight are essential; requires clear close & distance vision, peripheral & color vision, depth perception and ability to adjust focus. Position requires lifting to 25 lbs; on occasion more based upon patient demands.
  • Work Environment: Occasional (less than 1/3 of time) exposure to bloodborne pathogens that requires use of personal protective equipment. Office, clinic and hospital settings.
