Add to favorites Favorited View favorites Pay Grade/Pay Range: Minimum: $27.21 - Midpoint: $35.35 (Hourly N9) Department/Organization: 208401 - UMC - Clinical Normal Work Schedule: Monday - Friday 8:00am to 4:45pm Job Summary: The RN I provides professional nursing care to patients. Reviews physicians' orders for accuracy and authorization prior to implementing them. Prescribes and initiates nursing treatment through direct, assisted, or delegation to other staff. Additional Department Summary: Performs liaison duties for University Medical Center (UMC) clinic patients. Responsible for organization and execution of care management services including pre-visit planning, care gap analysis, and post-visit care coordination. Proactively manages Annual Wellness Visits at UMC clinics. Refers patients into the appropriate external care management programs. Communicates and connect with patients in-person, by phone, or electronically as needed. Required Department Minimum Qualifications: Active RN license in the State of Alabama in good standing. One (1) year of RN nursing experience. Must have a current CPR certification and have used an electronic health record system in a previous learning or employed role. Skills and Knowledge: Comprehensive clinical assessment skills in the areas of population and chronic disease management. Familiarity with current clinical practice standards for ambulatory and primary care. Ability to prioritize tasks and thrive in a busy, multi-practice environment. Familiarity with community and commitment to learning and understanding through on the ground networking, community assessment, etc. Preferred Qualifications: Bachelor of Science in Nursing (BSN) preferred. Experience in care coordination, care/case management, quality improvement, ambulatory care or community health.
