*A local Public Health Insurance Plan in Southern California is looking to hire a fully remote Bilingual Behavioral Health Navigator! *
*Description* * Assesses the needs of members and develops plans to facilitate access to care, including appointment scheduling, education, and care coordination with primary care providers, mental health professionals, county services, and community organizations. * Maintains accurate health records in compliance with state, organizational, and NCQA regulations, and in accordance with community standards. * Leverages resources from public and private agencies and community organizations to meet members' needs. * Provides support to members and their families by explaining treatment plans and benefits, offering guidance on supporting treatment, and implementing interventions to improve treatment acceptance and adherence. * Ensures coordination of care, patient education, and referrals based on member assessments, requests, or established policies and procedures. * Collaborates with community-based and government agencies to ensure continuous and coordinated care for members. * Participates in data collection and management activities. * Effectively manages multiple member referrals while maintaining daily productivity and meeting all assigned requests. * Participates in consultations, peer reviews, team activities, and meetings. * Performs other duties and responsibilities as assigned. *Skills & Qualifications* * Bilingual in English and Spanish Required * Knowledge and experience in mental health services, specifically within Medi-Cal and county care systems. * Capable of addressing the diverse needs of clients, gathering relevant care data to identify key issues, and managing high-volume client cases simultaneously. * Proficient in Microsoft Office Suite (Outlook, Word, Excel) with a high level of skill and efficiency. * Strong coordination skills for managing Care Management requests, with a clear understanding of the principles and practices required for effective hand-offs and follow-through with clinical teams. *Pay and Benefits* The pay range for this position is $30.00 - $35.00/hr. Eligibility requirements apply to some benefits and may depend on your job classification and length of employment.
