The Wounded Warrior Project (WWP) Independence Program (IP) Case Manager provides direct support and services to severely injured Warriors and their family members in the Independence Program. The IP Case Manager works with individuals with varying physical, mental, and cognitive challenges while maintaining a philosophy of individualized services designed to empower the Warrior and maximize independence and quality of life.
Note: The Independence Program is designed to help warriors' live life to the fullest, on their own terms. It serves wounded, ill, and injured veterans living with a moderate-to-severe brain injury, spinal cord injury, or other neurological conditions. The warrior's cognitive or physical challenges may limit his or her opportunity to access resources and activities in his or her community. The Independence Program helps break down these barriers.
DUTIES & RESPONSIBILITIES Caseload management:
Maintain a caseload of assigned Warriors. Encourage Warrior's to participate in the full suite of Independence Program Core Services (Community Support Specialists, alternative therapies, Continued Care Services, etc.). Complete intakes to determine eligibility into the Independence Program and provide recommendations regarding program enrollment. As required, completion of comprehensive assessments in the Warrior's home to determine service needs and development of appropriate care plans. Work in collaboration with the Warrior to develop individualized goals to improve independence. Identify, locate, and provide community supports and resources for Warriors in order to reintegrate and reconnect with their communities and increase their independence. Research programs, services, resources, recreation, transportation, education, day programs, etc. to provide appropriate resources to Warriors served in the Independence Program. As required, identify and contract with community resources to coordinate services for Warriors in order to reintegrate and reconnect with their communities and increase their independence. Monitor and appropriately utilize service hours provided. Monitor effectiveness of services provided to assess Warrior's progress on outcome goals. Monitor cost levels for assigned cases, identify trends, prepare reports, and provide recommendations for improving service delivery, impact of services, and strategies to increase reach and scope of service availability. Maintain regular communication with Warriors on the assigned caseload to ensure a best in class, high touch support and a positive warrior experience. Maintain and update all WWP electronic tracking systems, specifically the warrior and family member, spouse, and/or caregiver database (Salesforce).
Program management and development:
Ensure programmatic oversight and alignment of resources, services, and goals. Evaluate warrior funding requests and approve appropriate funding exceptions for essential services. Support Warrior engagement with WWP programs. Analyze program engagement data, identify programmatic trends, and provide regular status reports to leadership regarding caseload engagement in WWP services.
Resource management and development:
Cultivate and maintain relationships with local community resources to warriors on caseload. Ensure alignment of these resources with warrior needs. Assist with communication coordination among family members, caregivers, WWP program staff, other external organizations, agencies, and healthcare facilities. Provide IP briefs both externally and internally. Identify, develop, conduct, lead, and participate in special projects, e.g., event coordination and planning, training, curriculum development, research, presentations, mentoring, and other programmatic endeavors. Attend and actively participate in required training and/or meetings, including but not limited to New Teammate Orientation, WWP Cares, ASIST Suicide Prevention training, Leadership training, culture/team based training, or departmental huddles. Other related duties as assigned.

EXPERIENCE Requirements
Four years of experience working with military or veteran populations in support of their development and readjustment doing case management, managed case, community outreach, or mental health programming. Four years of experience working with and providing support to wounded service members who are dealing with both visible and invisible injuries, e.g., brain injury, post-traumatic stress disorder, and combat stress. Two years of experience conducting intakes. Proficient in Salesforce or other CRM system.

EDUCATION Requirements
Master's degree in social work, psychology, counseling, rehabilitation counseling, case management, or related mental health field.
