Position Summary Provides comprehensive psychological, social, and practical support to a designated caseload of patients and families, including conducting social work assessments, identifying problems, setting goals, and developing and implementing care plans. Coordinates continuity of care by making appropriate referrals to external agencies. Collaborates closely and provides leadership within a multidisciplinary team to optimize patient outcomes. Serves as a clinical resource and subject matter expert in a specialty area, offering clinical oversight and consultation in the absence of the Clinical Social Worker III or Director. Demonstrates a strong focus in clinical practice, education, or research, and functions as a mentor to peers. May be required to provide on-call coverage as needed. Competencies A. Education
B. Knowledge
Comprehensive understanding of biopsychosocial and cultural factors in clinical social work.
Familiarity with organizational systems, procedures, and objectives relevant to social work practice.
Strong knowledge of community resources and the current healthcare environment.
Ability to gather, synthesize, and analyze relevant data to inform patient care decisions.
Understanding of the medical and psychosocial impacts of illness on social functioning, specific to patient populations.
In-depth awareness of community resources aligned with patient needs, mental status, and functional limitations.
C. Skills
Excellent verbal and written communication skills to effectively interact with hospital staff, healthcare professionals, patients, and families.
Proficiency in delivering social work services in mental health or healthcare settings.
Strong analytical skills to assess patient needs, develop treatment plans, and implement effective interventions.
Ability to identify systemic barriers and recommend solutions to enhance patient care.
Capacity to work independently and provide complex counseling at both micro and macro levels.
Ability to mentor and support professional growth in others.
D. Licensure/Certification
Licensed by the Maryland Board of Social Work Examiners at the LCSW or LCSW-C level.
Exceptions: * Licensed to practice social work in another state or jurisdiction with comparable qualifications. * Meets Board requirements as defined by regulation. * Has a license application pending with the Board. * Has passed Board-prescribed examinations for the license sought.
