Job Description:
Position Title: Patient Care Coordinator General Description: Assists with the daily functions, patient outreach, and identification of community resources for patients, patient counseling and education for the multi-disciplinary clinics. Essential Responsibilities: Responsibilities listed in this section are core to the position. - Responsible for patient counseling and referrals as appropriate for patient needs. Assists with coordinating patient care. Responsible for the on-going daily management of patient continuity of care for patients entering and followed through the multi-disciplinary clinics. Organizes and assists with conducting community outreach on behalf of the Program.
- Maintains a customer service program to ensure a high quality service and customer satisfaction for patients in the clinical setting. Creates and conducts surveys for evaluating satisfaction of the users of the program. Participates in and initiates advancement of hospital/staff/community and patient relations. Survey and follows-up with referring providers to ensure high quality customer service. Meets with patients to ensure customer satisfaction with the Program and hospital experience.
- Utilizes and or creates databases for tracking and following patients through out-patient clinical settings. Organize and analyzes program data on a regular basis. Creates and maintains referring physician informational databases. Maintains all documentation and files on behalf of the program

Minimum Qualifications: Education: Associate's degree in health related field required. Bachelor's Degree in a Health related field (health education, social work, psychology, nursing, or related field preferred. Experience: One (1) year of experience in a medical or other clinical care setting required. License(s)/Certification(s)/Registration(s) Required: None. Knowledge, Skills and Abilities:
- Strong customer service skills
- Excellent verbal and written communication skills
- Proficient with the use of Microsoft Office tools
