Responsibilities
Wellstone Regional Hospital is a 100-bed acute care facility located in Jeffersonville, Indiana and has been providing quality health care to the residents of Southern Indiana and the Louisville area since 2003. Wellstone specializes in the treatment of Adolescents, Children, Adults, and the CD population. In addition to our in-patient services, we offer out-patient programs as well. The Step-Down Navigator is responsible for meeting with all eligible patients to discuss the benefits of Outpatient services as part of their continuum of care treatment. The Step-Down Navigator will be responsible for communicating eligibility to key members of senior leadership & associated departments and will be responsible for routinely collecting and entering data, specific to Outpatient eligibility, into Midas for trending report purposes. He/She will have a thorough understanding of the Step-Down Playbook, implementing components of the Playbook as needed. A thorough understanding of the Inpatient workflow process, as it pertains to case management and discharge planning, is preferred. Qualifications Job Duties: Consult with the Social Services, UM and Outpatient Departments on patient eligibility; Maintain current awareness of Outpatient programming; Maintains Midas data collection process; Identifies and communicates data driven trends in the step-down process; Review and report on data and processes routinely in Quality/PI meetings; Other duties as assigned. Requirements: Education: Bachelor's Degree in mental health/behavioral sciences field preferred. Experience: Minimum two (2) years experience in psychiatric/mental health field.
