MINIMUM QUALIFICATIONS:

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

1. Current Licensed Practical Nurse license issued by the state in which services will be provided or current multi-state Licensed Practical Nurse license through the enhanced Nurse Licensure Compact (eNLC).

2. Obtain certification in Basic Life Support within 30 days of hire date.

CORE DUTIES AND RESPONSIBILITIES:The statements described here are intended to describe the general nature of work being performed by people assigned to this position.They are not intended to be constructed as an all-inclusive list of all responsibilities and duties.Other duties may be assigned.

1. Collects any outside records that arrived overnight and ensures they get to medical records.

2. Obtains outside records as needed including getting release of medical information forms signed.

3. Obtains current medication lists from pharmacies, enters home medications into EPIC, and informs physician so that this can be reviewed.

4. Ensures every patient has a primary care physician on the first day of admission. If patient does not have primary care physician, provides list of primary care physicians who are accepting new patients in the patient's home county, and then follows-up the next day regarding which PCP was chosen and enters information into EPIC.

5. Works with Care Management team to develop a list of primary care physicians in each county that are accepting new patients and updates list every six months.

6. On the day prior to anticipated patient discharge, Sets up follow-up with any external PCP's and specialists and any external testing that needs to be performed.

7. On the day following discharge (if not going to a nursing home), Calls Patient and/or family and confirms the following: Ensures they understand their medications, Responds to teaching opportunities to enhance patient's compliance and safety, Ensures they have been able to get all of their medications, Has them repeat back the discharge plan, Finds out if any other concerns or questions, if any discrepancies/questions, checks in with physicians., and Communicates back with patients or pharmacies regarding the plan.

8. Ensures that all patients discharged over the weekend have a follow-up appointment, and schedules one if not.Calls patients as if it were day after discharge follow-up.

9. Assists administrative assistant with any clinical calls that come in.Ensures that patient's be appropriately referred to PCP in a question not directly related to the hospitalization.

10. Assists with sorting through home health orders, ensuring that they get to the appropriate person, discharges repeat requests if already completed, and refers to PCP as appropriate.
