Summary:
The Outcomes Specialist role provides oversight to visiting clinicians by ensuring appropriate home health coding, accurate OASIS assessments and complete supporting documentation.

Required Minimum Knowledge, Skills, and Abilities (KSAs)

  1. Education: Bachelor's Degree in appropriate clinical designation/field with three years of clinical experience or education as a RN, PT, OT, or SLP with five years of clinical experience. One year of experience must be in home care.
  2. License/Certifications: Valid Driver's License required with liability insurance as specified by agency policy. Current BLS Certification or must obtain within 30 days of start date. OASIS certification or must obtain within six months of start date.
  3. Experience: See Education.
  4. Additional Skills/Requirements Required: N/A
  5. Additional Skills/Requirements Preferred: Demonstrated competency in OASIS and home health coding.
