Position Summary
The FIT Program Community Health Worker will provide outreach, counseling, navigational, and case management services to individuals recently released from incarceration with chronic medical, psychiatric and/or substance use conditions who present at or are recruited by the FIT Program. This position is based at the UNC REACH (Recovery and Access to Comprehensive Health) Enhanced Primary Care clinic and Oak City Cares in Raleigh.
Minimum Education and Experience Requirements
Demonstrated possession of the competencies necessary to perform the work.
Required Qualifications, Competencies, and Experience

  • Willingness to work in various environments, including prison/jail settings, shelters, and home visits
  • Knowledge of providing information, education, intervention and/or referral services to culturally diverse populations and/or community health worker certificate/CASAC
  • Comfortable working with culturally diverse populations and a diverse array of formerly incarcerated people including those who have committed a sex offense
  • Manual dexterity to operate office equipment, including keyboard, copier, telephone
  • Must be able to view and work on a computer for a long period of time, ability to use Microsoft Office and internet
  • Must be able to work in stressful environment
  • Ability to work flexible job hours
  • Position requires prior justice system involvement or justice history
  • Valid NC driver's license and access to reliable transportation

Preferred Qualifications, Competencies, and Experience

  • Experience working with health care systems, mental health and substance use treatment programs, community-based organizations.
  • Experience within the past 5 years working as part of a multi-disciplinary team, collaboratively practicing with therapists, psychiatrists, PCPs, health workers, health educators, and/or other health professionals.
  • Experience with client assessment, treatment planning, conducting interviews/investigations, advising/counseling clients, and participating in health promotion and health education activities.
  • Excellent organization, follow through, and ability to juggle multiple priorities in fast-paced environment with multiple collaborators
  • Excellent verbal and written communication skills.
  • Bilingual English/Spanish
  • Experience living or working in Wake County, NC
