Community Health Worker - Diabetes

Santa Barbara Cottage Hospital seeks a Community Health Worker - Diabetes for their CH Population Health department responsible for serving as a trusted resource for patients living with diabetes, their families, and community members. This position provides lived experience support with health education, resource and care navigation, referral, and care coordination for patients with type 2 diabetes in hospital and community settings. Responsibilities include support for patients through the discharge process, facilitating resource connections for social needs, patient goal setting, and follow-up care post-discharge. Additional responsibilities include:



  • Completing basic health and social needs assessments, digital documentation, and verbal and written communication for patients and their caregivers.
  • Lived experience with diabetes and an understanding of the patient challenges is essential.


This position reports to the Manager of Community and Parish Nursing and works closely with varying departments within Cottage Health.

QUALIFICATIONS:

All job qualifications listed indicate the minimum level necessary to perform this job proficiently.

Education:



  • Preferred: Completed Community Health Worker or Promotor/a training or certificate.


Certifications, Licenses, Registrations:



  • Minimum: Valid California driver's license, reliable transportation, and insurance coverage as required by California state law.
  • Preferred: CHW, Promotor/a, or medical assistant certification.


Technical Requirements:



  • Minimum: Competency, knowledge, and lived experience with a diabetes diagnosis and ability to connect with patients with diabetes. Ability to type and navigate on a computer.
  • Preferred: Bilingual in Spanish and English. Ability to write, read, and speak in English for patient documentation.


Years of Related Work Experience:



  • Minimum: Personal lived experience with a diabetes diagnosis.
  • One (1) year of experience working in a community health or related service, including volunteer or internship positions in a community setting. Diabetes education or training.
