Reporting to the Clinical Supervisor of the team, the Medicare Care Management Field Coordinator supports the health and well-being of San Francisco Health Plan (SFHP) members by addressing health needs and social drivers of health, both individually and across communities. As part of the Care Management team, they work directly in the community with those who benefit most from SFHP services.
