The Supervisor, Case Management has core responsibilities to providing leadership and supervision to case managers, social work case managers and transition coordinators. The lead role provides oversight of group functioning, assignments and provides clinical supervision to staff in day to day assignments. The Supervisor provides organization and support to the orientation of new staff, incorporation into the hospital and the department as a whole and provides leadership and direction as needed to all members. The Supervisor brings issues forward that are central to the role and contributes to the improvement and ongoing development of core critical processes. The supervisor utilizes principles of patient and family centered care to direct interventions and changes in practice. The position is responsible for continually identifying new opportunities to promote professional development and success as an interdisciplinary member of Yale New Haven Health staff, bridging throughout the continuum for the quality care of all patients.

Responsibilities

Qualifications

EDUCATION

RN required. Minimum of Bachelors required in a clinically related field. Masters Preferred.

EXPERIENCE

At least four years as a Care Manager in excellent standing.

LICENSURE

RN licensure in CT, Case Management Certification strongly preferred.

SPECIAL SKILLS

Leadership, critical thinking, understanding of group dynamics.

PHYSICAL DEMAND

Weekend and holiday requirements as needed.
