Job Title: Community Health Worker I (CHW I) Position Overview: Join our team as a Community Health Worker I (CHW I), where you'll play a vital role in enhancing patient care by addressing social determinants of health. Reporting to the Manager of Care Management, you will provide bedside and community support to patients, ensuring they receive comprehensive care from admission through post-discharge. Your responsibilities will include conducting SDOH screenings, enrolling patients in community programs, delivering patient education, coordinating appointments, and engaging with patients to reinforce treatment plans. As a key collaborator with physicians, nurses, care managers, and community partners, you will actively participate in community engagement activities and assist with volunteer coordination. Reliable transportation is essential for home visits and community outreach. Key Responsibilities:

Compensation will be commensurate with equity and experience for roles of similar scope and responsibility.
