Social Worker - Case Management - Full Time - Days General Summary Full Time Benefits Eligible Schedule: 7:30am - 4:00pm The Case Management Social Worker coordinates the overall interdisciplinary plan of care for patient, from admission to discharge. Responsibilities Creates plan for care across the continuum, integrating patient/family preferences and values Serves as a resource to assess, evaluate, recommend and treat legal, neglect, substance abuse, crisis, psychosocial and various adjustment needs Advocates for resources and removal of barriers Maintains ongoing dialog with supervisor and care transition team members to ensure effective implementation and reevaluation of health plan Acts as a resource for adequate medical record documentation, complex level of care recommendations, and services as they relate to diagnoses, and treatment options for post-discharge care Licensure / Certification / Registration One or more of the following required: Counselor credentialed from the Michigan Board of Counseling obtained prior to hire date or job transfer date Social Worker credentialed from the Michigan Board of Social Work obtained prior to hire date or job transfer date, any level accepted Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred Education Bachelor's degree required Master's degree preferred Behavioral Health experience preferred Additional Information Organization: Henry Ford Health St. John Hospital Department: Social Service 001 Shift: Day Job Union Code: Not Applicable
