Overview

Under the supervision of the Coordinator or Supervisor, provides an enhanced level of support for persons experiencing early episode psychosis utilizing the Navigate model. The NAVIGATE Program is an innovative treatment program that serves adolescents and young adults who are within two years of experiencing the onset of psychosis.

The Case Manager & Supported Education/Employment Specialist is a dual-role position within the Coordinated Specialty Care (CSC) team. This position provides holistic case management, resource navigation, and specialized support for clients pursuing educational or vocational goals during recovery from a first episode of psychosis.

The role emphasizes strengths-based, youth-driven, and recovery-oriented care, supporting clients to maintain or return to school/work as part of functional recovery. Collaboration with therapists, psychiatrists, peer specialists, family partners, and community agencies is essential.

Responsible for carrying out all activities of the program/sub-unit in such a manner that fulfills RESTORE's mission, policies and procedures.

Responsibilities
  • Conduct strengths-based assessments of psychosocial needs (housing, benefits, health care, daily functioning).
  • Develop and update individualized service plans.
  • Provide community-based support, life-skills coaching, and linkage to resources.
  • Assist clients in navigating medical, behavioral health, financial, and social systems.
  • Provide individualized school and work support aligned with individual placement and support -informed practices.
  • Assist with job search, resumes, interviews, workplace coaching, and employment retention.
  • Support academic planning, enrollment, coursework, disability services, and academic accommodations.
  • Build connections with employers, vocational programs, training providers, and educational institutions.
  • Serve as liaison among clients, families, internal CSC staff, and external systems.
  • Coordinate communication with schools, employers, hospitals, outpatient providers, and community agencies.
  • Participate in transition planning, including hospital discharges and level-of-care changes.

Qualifications
  • A minimum of one (1) years' experience interacting with persons with emotional impairments
  • Familiarity with Employment benefits and continuing education programs
  • Bachelor degree in human services or related field preferred

Experience with one of the following preferred:

  • Early psychosis programs or serious mental illness
  • Supported Employment/IPS or Supported Education models
  • Youth/young adult development, college counseling, or career services
