The Director will lead the establishment and integration of the Universal Nurse Home Visiting ' Community Health Worker Program to ensure ongoing programmatic excellence and fidelity to the Family Connects model. They will have overall strategic and operational responsibility for all areas of the program including: Program Planning & Operations, Financial Planning & Management, Fundraising & Development, Grants & Contracts, Human Resources, Marketing & Outreach, Program Quality Improvement, Risk Management, and Stakeholder & Community Relations. They will demonstrate effective administration to ensure clinical services and program activities that meet the accreditation standards set forth by Family Connects International as well as meet the regulations and contracted deliverables of the program's funding partners and other stakeholders. They will provide leadership in the establishment and maintenance of a culture of support and accountability within the program.

Responsibilities

  • 1. Program Planning & Operations
    • 1.1 Develop, implement, and track an operational plan to ensure that the clinical services meet the needs of the children and families served, and that the operation and outcomes of the program meet the requirements and expectations of The Governance Team, Family Connects International, and YNHHS standards of care.
  • 2. Financial Planning & Management
    • 2.1 Act as a liaison to the YNHHS Finance Department and the Grants Resource Office financial liaison.
  • 3. Fundraising & Development
    • 3.1 Secure adequate funding and resources for the program.
  • 4. Grants & Contracts
    • 4.1 Work closely with the YNHHS Grants Resource Office to manage the State contract with the Office of Early Childhood.
  • 5. Human Resources
    • 5.1 Serve as the liaison to the YNHHS Human Resources Department.
  • 6. Marketing & Outreach
    • 6.1 Develop and maintain a marketing and outreach plan for the UNHV-CHW program.
  • 7. Program Quality Assurance
    • 7.1 Ensure that the services offered by the UNHV-CHW program benefit the Greater Bridgeport community and reflect the vision, mission, and priorities of Family Connects International, Bridgeport Hospital, and YNHHS.
  • 8. Risk Management
    • 8.1 Establish and maintain a positive, healthy, and safe work environment in accordance with all appropriate legal and YNHHS requirements and regulations.
  • 9. Stakeholder & Community Relations
    • 9.1 Serve as the lead representative and spokesperson for the UNHV-CHW Program in the community and in relevant meetings, activities, and collaboratives.

Qualifications

EDUCATION

Bachelors degree required, Masters degree preferred. Preferred areas of study include Social Work, Child Welfare, Nursing, Public Health, and Healthcare Administration.

EXPERIENCE

Leadership experience within the non-profit sector preferred. Experience developing and implementing a pilot program within the prescribed guidelines and requirements in a diverse community highly valued. Experience in the maternal, infant, and early childhood home visiting highly valued.

LICENSURE

None required.

Proficiency with Microsoft Office programs (Word, Excel, PowerPoint, Microsoft Teams, Edge) and web-based programs. Knowledge of EPIC and SharePoint highly valued.

PHYSICAL DEMAND

Work Location(s): Remotely, in the hospital system, and in community locations. Valid driver's license and use of reliable personal vehicle with appropriate insurance required.
