ChristianaCare's Psychosocial Oncology department at the Helen F. Graham Cancer Center is looking to hire a Full Time Behavioral Health Therapist II- Psychosocial Coordinator.

This clinical team member will provide high-quality professional social work services that enhance the quality of patient care at Christiana Care through the provision of psychosocial screening and intake, interdisciplinary consultation, complex care coordination, resource identification and navigation, development and delivery of support and psychoeducational groups, and educational services for patients and caregivers affected by cancer. They will provide ongoing clinical and programmatic collaboration with the Psychosocial Oncology program within Behavioral Health and the Helen F. Graham Cancer Center & Research Institute in support of an integrated model of health care.

Work Schedule:

  • Monday- Friday: 8am- 4:30pm.

  • Opportunity to work a hybrid schedule available!

  • Hybrid schedule includes, on average, three (3) days on-site and two (2) days remote. This is ad hoc per departmental needs.

  • No weekends/ holidays

Whether a position can be supported for remote work will be assessed based on whether ChristianaCare is able to meet the business requirements of the proposed remote location.

Key Responsibilities:

  • Provides coordination of psychosocial care and screening and navigation services for Psychosocial Oncology, in collaboration with Behavioral Health, Helen F. Graham Cancer Center, and affiliated medical practices.

  • Collaborates with multidisciplinary team (psychologists, psychiatrists, oncologists/surgeons, nurses, case management, support staff) regarding patient needs and facilitates connection to ChristianaCare and community-based services as appropriate.

  • Conducts timely brief phone consultations, triages incoming patient referrals, and conducts semi-structured interviews to connect cancer patients and their caregivers with appropriate psychosocial treatment.

  • Collaborates with Director of Psychosocial Oncology and clinicians to develop and deliver group therapy and psychoeducational services.

  • Effectively manages risk and provides crisis intervention services and resources when indicated.

  • Conducts semi-structured clinical intakes to support timely access to care based on clinic need (as needed).

  • Collaborates with internal and external groups and resources on programmatic initiatives in the service of current departmental initiatives.

  • Provides brief psychosocial screening and education in multidisciplinary centers to facilitate team-based and patient-centered care.

  • Communicates diagnostic impressions to multidisciplinary team and provides feedback to patients as requested by team.

  • Possesses general knowledge of evidence-based and integrative psychosocial interventions for anxiety and mood disorders, and other psychosocial reactions to cancer diagnosis/treatment, survivorship, and grief/bereavement to inform appropriate patient education, referral disposition, and triage.

  • Provides coordination of care services to patients, caregivers, and the multidisciplinary team.

  • Provides education, consultation, and advocacy to care team on behalf of patients.

  • Identifies and connects patients with community and government resources; advocates on their behalf as needed.

  • Performs other related duties as required.

Qualifications:

  • Master's or doctor's degree which qualifies in their state for licensure or certification is required

  • Licensed Clinical Social Worker, or LCSW preferred

    Marriage and Family Therapists (MFTs) and Mental Health Counselors (MHCs) with at least 2 years or 3,000 hours of postmaster's degree clinically supervised experience required and must be licensed to bill independently.

  • 2+ years of current or recent experience in a health care setting

  • Prior psycho-oncology and/or palliative care training or experience preferred.

Hourly Pay Range: $32.77 - $52.43 This pay rate/range represents ChristianaCare's good faith and reasonable estimate of compensation at the time of posting. The actual salary within this range offered to a successful candidate will depend on individual factors including without limitation skills, relevant experience, and qualifications as they relate to specific job requirements.
