Description

Providence St. John's Health Center is seeking a full-time Clinical Pharmacist Specialist for active involvement in medication management through direct patient care activities in the ICU specialty area.

Serve as a content expert and therapeutic consultant. Apply knowledge and critical thinking skills to evaluate, optimize, and collaborate with the healthcare team to ensure cost-effective, patient-centered medication therapy. Establish effective relationships to provide high quality care and provide leadership within the healthcare team for provision of clinical pharmacy services in a specific practice area, demonstrating professionalism and exhibiting the core values of Providence.

Provide effective and targeted education related to medication therapy to the health care team and patients. Communicate and collaborate with caregivers across the continuum during transitions of care. Functions as the medication expert for the care team in areas that include cost effective, evidence based, appropriate use of medications, clinical pharmacology, pharmacokinetics, and medication therapy management while adhering to all clinical practices approved by P&T, as appropriate for practice setting.

Participate in interdisciplinary committees, patient care teams, medication protocol reviews and medication use evaluations related to area of practice. Understand the clinical and financial ramifications of services provided & understands the principles of formulary management and follows formulary procedure.

Lead development, implementation, and advancement of pharmacy services in the ICU specialty area (e.g., medication safety initiatives, protocol development, formulary changes/recommendations, alternative medications in response to changes/shortages and changes in response to regulatory requirements).

Interact and provide oversight of students to workflow and practice setting, serving as a primary preceptor and mentor other preceptors on the pharmacist team. Assist in medication safety activities, reporting, and regulatory readiness. Support quality initiatives and programs through modelling behavior and coaching peers. Monitor for barriers and coordinate educational support of staff with management team.

Review and interpret provider orders to assure appropriateness. Optimize medication therapy, including but not limited to checking for medication interactions, patient allergies, and therapeutic duplication. Utilize pharmacokinetic principles and patient clinical status to establish appropriate medication regimens. Document all clinical monitoring and interventions as outlined in departmental policies. Perform duties of pharmacist as needed/assigned.

Assume personal responsibility to maintain professional development and education in clinical pharmacy practice in order to act as a role model and resource to pharmacy staff. Actively participate in professional organizations and serve as a pharmacy department representative on regional/system committees. Stay up to date with new clinical guidelines and educate other caregivers regarding best practice changes.

Maintain responsibility for completion of all assigned daily work and documentation. Ensure appropriate clinical handoff occurs with the oncoming shift. Lead, review, and provide feedback on the work of supportive personnel. Perform other duties as assigned.

Required Qualifications:



  • Doctor of Pharmacy (PharmD)
  • PGY2 (Postgraduate Year Two) residency training - or - PGY1 (Postgraduate Year One) residency training and 2 years of experience within ICU specialty practice setting - or - 4 years of experience in ICU specialty practice setting)
  • California Registered Pharmacist License
  • BPS certification by the Board of Pharmacy Specialties in Pharmacotherapy
  • Basic Life Support-Provider (BLS)
  • Advanced Cardiac Life Support (ACLS)
  • Requires skills and experience in the following:
  • Experience with health information systems, electronic health record (EPIC), medication information resources and databases necessary for direct patient care
  • Experience with Pyxis, IV Admixture/Pump Equipment, and Point of care equipment
  • Experience with clinical pharmacy knowledge base, including independent ICU clinical decision-making and practical ICU knowledge
  • Experience independently managing ICU pharmacotherapy decisions, including dosing, titration, and monitoring of high-risk medications

The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
