SUMMARY:

Under the supervision of the Program Manager, provides case management, rehabilitation, crisis interventions, and outreach services in home, office, and community settings to adults with serious mental illness. Collaborates with psychiatric and allied mental health team in engaging clients in assessment, diagnosis, intervention and treatment.

MAIN DUTIES AND RESPONSIBILITIES:

  • Provides clinical interventions such as needs assessment, resource navigation, skill teaching, and care coordination to adult clients in community support program.
  • Assertively engages and builds motivation in clients to engage in treatment services.
  • Provides majority of services in community settings, which may include homes, businesses, or outdoor spaces.
  • Transports clients in personal vehicle as clinically appropriate.
  • Completes detailed, accurate, and timely documentation in the electronic medical record for all services provided following established protocols.
  • Provides ongoing assessment of client's functional status and progress toward treatment goals and ensures changes are integrated into treatment plan.
  • Assists clients in obtaining and utilizing insurance and other benefits, medical care, employment, housing, and social support network.
  • Assists clients in skill building for activities of daily living (ADLs) to increase and stabilize level of functioning.
  • Conducts screenings and assessments within scope, and develops interventions for assessed needs.
  • Provides psychoeducation to clients and their family members on mental illness, substance use, community resources, and other topics to increase adherence to treatment plan and increase client functioning.
  • Participates in supervision, team conferencing, department, and agency meetings.
  • Coordinates, and monitors treatment services for assigned cases, which includes but is not limited to:
    • Participating in treatment planning with client and interdisciplinary team and ensuring treatment plan is updated and implemented following established protocols.
    • Assists with discharge planning and coordination with other treatment providers.
    • Utilizes data tools to ensure screenings and documentation are completed correctly and on time following established protocols.
  • Performs other duties and responsibilities as assigned or needed.

QUALIFICATION / REQUIREMENTS:

  • Associate's degree in human service field required. Bachelor's degree in related field preferred.
  • Experience or interest working with individuals living with mental illness, substance use disorders, and/or psychosocial stress.
  • Bilingual fluency in Spanish/English or other critical language preferred.

NECESSARY SPECIAL REQUIREMENTS:

  • Knowledge of human development and family systems issues.
  • Understanding of mental health and substance use diagnoses
  • Knowledge of community resources for clients and families with behavioral health disorders.
  • Advocacy and coaching skills.
  • Valid driver's license and use of vehicle to travel during work hours to perform the major functions of the job.
  • Personal vehicle in working order with valid registration, auto insurance that meets agency minimums, and ability to qualify under agency requirements for driving record.
  • Ability to work collaboratively as a team to coordinate care for clients with intensive behavioral health needs.
  • Ability to communicate effectively and professionally both in writing and verbally with clients, family members, supervisors, other agency staff, community professionals and other members of the community.

Pay Range:

$19.97-$32.96
