REPORTS TO: Home Health and Hospice Clinical Manager

GENERAL DESCRIPTION

The Supervisor for Home Health manages employees to assure coordinated, interdisciplinary care to patients in their home setting. Participates in development of department policies, procedures, implements quality management plan and evaluates for compliance. Responsible for clinical standards including targeted QAPI outcomes. The Supervisor performs all duties in a manner that promotes team concepts and reflects MultiCare's Mission, Vision and Values.

PRINCIPAL ACCOUNTABILITIES

- Demonstrates services excellence and positive interpersonal relations in dealing with others, including patients, families. Caregivers, employees, all leadership, medical staff, volunteers and community members to assure positive relations are maximized.

- Develops individual objectives consistent with the MultiCare Health System's Patient Satisfaction, Outcomes and Publicly reported measures.

- Counsels and provides performance management to employees. Holds staff accountable for patient outcomes.

- Facilitates communication, coordination and problem solving with physicians, patients, families, staff and payors. Serves as a liaison to hospital, acute care managers and community agencies.

- Participates with the Home Health Manager and Director, Quality Manager, Education Coordinator and others in developing, evaluating and revising home care policies, procedures, standards, competencies, quality and risk management and objectives to assure they cover clinical operations and processes to meet industry standards and regulatory requirements.

- Supports coordination of admission between Home Health Access and Home Health Scheduler by daily oversight of staffing functions to provide service to those in our community.

- Oversees quality of documentation to assure all regulatory and quality measures are met.

- Age related competency for Pediatric, Adolescent, Adult and Geriatric patients.

- Oversees staff providing direction and guidance, and administering management functions within the provisions of MultiCare policies and standards and federal, state and local regulations.

- Adheres to MHS Attendance and Punctuality Policy and Procedure standards. Maintains reliable attendance.

MINIMUM QUALIFICATIONS

KNOWLEDGE, SKILLS, & ABILITIES

- Knowledge of the principles of Home Health & Hospice in order to effectively analyze data and make decision

- Knowledge of Home Health clinical standards and practices, Medicare/Medicaid/ Joint Commission requirements, supervisory principles and practices

- Skill in directing the work and activities of staff with the ability to delegate effectively

- Skill in providing leadership and in the mentoring of staff

- Skill in time management and ability to meet deadlines

- Skill in creating high-performing teams

EDUCATION & EXPERIENCE

- Associates degree; Bachelors preferred

- Current RN License in the State of Washington

- Home health supervisory experience

- 2 years of clinical professional experience

- Current BLS (Basic Life Support for Health Care Provider) is required within 30 days of hire.

- Oasis Certification and EPIC experience preferred.

Position Summary

The Supervisor of Home Health Clinical is an excellent leadership opportunity overseeing consistent delivery of quality home health care and services. You will utilize independent clinical decision-making capacities every day while engaged in dynamic and ever-changing work projects.

Requirements

  • Current Registered Nurse, Physical Therapist, Occupational Therapist, or Speech Language Pathologist license in Washington State
  • If Registered Nurse: Registered Nurse license (RN) in Washington State or multi-state licensure endorsement (MSL): Bachelor's degree in Nursing, or obtained within five (5) years from the initial date of hire; RN with 15+ years of service or internal candidates hired before January 1, 2021 may be considered in satisfaction of this requirement in lieu of an education agreement
  • Master's degree preferred
  • Current BLS Certification required
  • Three (3) years recent Home Health experience
  • Previous experience and demonstrated skills in a supervisory or lead capacity

Pay and Benefit Expectations

We provide a comprehensive benefits package, including competitive salary, medical, dental and retirement benefits and paid time off. As required by various pay transparency laws, we share a competitive range of compensation for candidates hired into each position. The pay scale is $104,665.60 - $150,612.80 USD. However, pay is influenced by factors specific to applicants, including but not limited to: skill set, level of experience, and certification(s) and/or education. If this position is associated with a union contract, pay will be reflective of the appropriate step on the pay scale to which the applicant's years of experience align.
