UC San Diego Health is seeking an outstanding pharmacy technician to join our Transitions of Care team, playing a vital role in ensuring accurate medication histories for patients admitted across our hospitals. Under general supervision, the Pharmacy Technician is responsible for obtaining, documenting, and maintaining accurate best possible medication histories (BPMH) for patients according to standards established by the Department of Pharmacy. This role supports transitions of care initiatives by ensuring high quality medication histories are collected upon admission to promote medication safety and continuity of care. The Pharmacy Technician will work collaboratively with pharmacists, providers, nursing staff, and interdisciplinary teams to identify and resolve discrepancies in patient medication histories. Responsibilities include interviewing patients and caregivers, contacting outpatient pharmacies and providers, reviewing external records, and documenting findings in the electronic health record. This position plays a critical role in reducing medication errors and supporting safe patient transitions. The ideal candidate is a self starter who is accountable, detail oriented, and able to work with minimal supervision in a fast paced healthcare environment. KEY RESPONSIBILITIES:
Pay Transparency Act Annual Full Pay Range: $67,756 - $84,209 (will be prorated if the appointment percentage is less than 100%) Hourly Equivalent: $32.45 - $40.33 Factors in determining the appropriate compensation for a role include experience, skills, knowledge, abilities, education, licensure and certifications, and other business and organizational needs. The Hiring Pay Scale referenced in the job posting is the budgeted salary or hourly range that the University reasonably expects to pay for this position. The Annual Full Pay Range may be broader than what the University anticipates to pay for this position, based on internal equity, budget, and collective bargaining agreements (when applicable). | |
