Position Summary

The Supervisor of Outreach Activities oversees regional outreach staff and processes to expand awareness, understanding, and access to Home Health and Hospice services. This role ensures consistent, highquality community engagement, referral development, and education across multiple service areas. The Supervisor collaborates with internal teams and external partners to strengthen referral pathways, support strategic growth, and promote exceptional patient and family experiences.

Responsibilities:

  • Supervise Outreach Coordinators across multiple regions, providing coaching, performance feedback, and ongoing professional development.
  • Oversee daily outreach operations, ensuring consistent execution of community engagement, education, and referralbuilding activities.
  • Implement and communicate departmental policies, procedures, and standards to ensure compliance and operational consistency.
  • Monitor outreach metrics, referral trends, and engagement data to evaluate effectiveness and identify opportunities for improvement.
  • Coordinate staff schedules based on program needs, community events, and strategic priorities.
  • Build and maintain strong relationships with providers, facilities, community organizations, and other referral sources.
  • Collaborate with Home Health, Hospice, Intake/Admissions, and Leadership to streamline referral processes and support service growth.
  • Ensure accurate documentation and reporting of outreach encounters, partner interactions, and community engagement outcomes.
  • Participate in short and longrange planning, quality improvement initiatives, and strategic outreach projects.
  • Support budgeting activities related to outreach operations, events, and materials.
  • Represent the organization at community events, health fairs, and professional meetings to promote Home Health and Hospice services.

Requirements:

  • Associate degree required; two (2) years of leadership experience may be considered in lieu of degree
  • Two (2) years of experience in Home Health, Hospice, community health outreach, care coordination, or related healthcare field
  • Previous experience and demonstrated skills in a supervisor or lead capacity
  • Knowledge of Home Health and Hospice regulations, referral processes, and service models preferred
  • Driver License/Abstract - Multiple Issuers
  • Other: vehicle for work business activities

Pay and Benefit Expectations

We provide a comprehensive benefits package, including competitive salary, medical, dental and retirement benefits and paid time off. As required by various pay transparency laws, we share a competitive range of compensation for candidates hired into each position. The pay scale is $94,889.60 - $136,572.80 USD. However, pay is influenced by factors specific to applicants, including but not limited to: skill set, level of experience, and certification(s) and/or education. If this position is associated with a union contract, pay will be reflective of the appropriate step on the pay scale to which the applicant's years of experience align.
