ChristianaCare's Supportive and Palliative Care program is seeking a compassionate, dependable, and detail-oriented Social Worker II-MSW who has the ability to build trusting relationships with our patient population and has a deep commitment to patient-centered outcomes to support our Wilmington and Newark locations. The Social Worker II- MSW in the Supportive and Palliative care program assist patients and families to identify social, emotional, and environmental needs and to connect them with available resources/ services through the provision of a full spectrum of social work services. Essential functions are fundamental, core functions common to all positions of medical social work, in providing case management services to patients in a medical social services program area including chronically ill patients and/or patients undergoing cancer treatment. The Social Worker II- MSW will supporting our patients in both the inpatient side of the program as well as providing outpatient services. Work Schedule:

**This is the advanced level responsible for directly providing specialized case management services and for social services consultation services to patients and professional medical care staff in the public and private sector. This level is differentiated from the lower level of this series by the greater independent judgement and the emphasis on providing consultative services.
