Colorado Statewide Traveler - Nurse Practitioner

Optum Care at Home is seeking a Nurse Practitioner to provide home-based medical care to medically complex patients across Colorado. This role focuses on managing chronic conditions, preventing unnecessary hospitalizations, and delivering comprehensive, relationship-based care in patients' homes.

• $40,000 Student Loan Repayment or $25,000 Sign-On Bonus
• Statewide traveler role with approximately 85% travel throughout Colorado
• All travel expenses covered
• Base salary plus incentive opportunities
• Home-based care for medically complex and geriatric patients
• Conduct comprehensive geriatric assessments and urgent visits in the home
• Order and interpret labs, x-rays, and EKGs
• Collaborate with PCPs and specialists to coordinate care
• Focus on improving quality outcomes, patient satisfaction, and reducing hospitalizations
• Quality-based compensation model, not productivity-based

• Certification through the American Academy of Nurse Practitioners (AANP) or American Nurses Credentialing Center (ANCC)
• Current, unrestricted NP licensure in the state of Colorado
• Current, unrestricted RN licensure in the state of Colorado
• 2+ years of clinical experience, primary care, home-based medical care, geriatrics or palliative care or emergency/urgent care
• Driver's License and access to reliable transportation; to travel up to 85% within Colorado (with notice, based on business needs) to complete home visits if you are driving a vehicle, you must comply with all the terms of the Optum Motor Vehicle Safety policy

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $89,900 to $160,600 annually based on full-time employment. We comply with all minimum wage laws as applicable.
