Optum Home & Community Care is seeking a Per Diem Nurse Practitioner to provide care for medically complex patients in home and community settings. This role focuses on delivering comprehensive, patient-centered care while helping improve quality outcomes, reduce hospitalizations, and support patients where they live.

Position Highlights
  • Per Diem schedule; Weekday M-F availability between 8a-8p preferred
  • $5,000 Sign-On Bonus
  • No call and no holidays required
  • Serve patients throughout the Bangor, Waterville, and Augusta regions
  • Conduct comprehensive health assessments and ongoing care management
  • Collaborate with interdisciplinary care teams and primary care providers
  • Focus on chronic disease management, preventive care, and care coordination
  • Opportunity to provide care in home and community-based settings
  • Autonomy with strong clinical support
Required Qualifications
  • Current, unrestricted NP licensure
  • National Board Certification as a Nurse Practitioner
  • Active DEA license or ability to obtain
  • Active driver's license and reliable transportation
  • Ability to travel within the assigned territory
  • Experience managing complex adult or geriatric patients

Compensation for this specialty generally ranges from $53.00 - $79.00 per hour. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. We comply with all minimum wage laws as applicable.
