Founded in 1999, Progressive Community Health Centers is an independent non-profit organization that operates four clinic locations in Milwaukee, Wisconsin. More than 100 employees support our agency's mission to improve the health and quality of life of the community by providing culturally competent services that address identified needs. We provide high quality, comprehensive care to roughly 16,000 patients annually, majority of who are low-income. No one is ever turned away from our clinics based on insurance status or ability to pay. We value a patient-centered philosophy and provide range of services that include Family Medicine, Internal Medicine, Women's Health, Urgent Care, Dental, and Integrated Behavioral Health. We also offer supportive services such as case management, health education and benefits enrollment.
Job Summary The RN/LPN works collaboratively as an active member of the interprofessional team to provide therapeutic patient and family centered care. The RN/LPN systematically and continuously collects and assesses data related to patient health status to develop, execute and evaluate the plan of care. The RN/LPN utilizes evidence to drive clinical decisions and practice related to individual and population health. The RN/LPN establishes goals and strategies for meeting care coordination needs across settings with the patient, family, and/or other care providers. The RN/LPN considers factors related to safety, effectiveness and fiscal accountability in planning and delivering care. The RN/LPN participates in continuous quality improvement initiatives and assumes responsibility for their professional development. ESSENTIAL FUNCTIONS Service - Provides coordinated, patient- and family-centered nursing care by actively collaborating with the interprofessional team to meet care needs across settings.

EDUCATION AND EXPERIENCE: Graduate of an accredited nursing program require. Currently licensed in the State of Wisconsin required. Previous experience with patients of all age groups required. Experience within a community health center and with EPIC preferred. Two years of general nursing in a clinic or hospital setting strongly preferred. It is mandatory that all employees, employed providers and volunteers are vaccinated. All hired candidates must submit proof of vaccination prior to their orientation. This is a mandatory requirement for Progressive Community Health Centers. The purpose is to minimize exposure to and transmission of the influenza virus in the workplace by providing occupational protection to employees and thereby preventing exposure to members of the community, which we serve.
